Telephone Consults with Dr. Hain

Dr. Hain is a Professor of Neurology, Otolaryngology and Physical Therapy at Northwestern University, in Chicago Illinois, USA. Dr. Hain has published more than 100 papers and chapters on dizziness and hearing disorders.

While Dr. Hain prefers to see patients in person, this is not always possible and this page describes how to consult with Dr. Hain on complex otoneurological issues via the telephone.

    Steps to set up a telephone consult.

  • Patient indicates interest in telephone consult (usually via email).
  • Appropriate questionnaire is completed and emailed to reception desk.
  • Other records are sent (email, fax, or regular mail). Radiology records (of brain and neck) are best sent via CD. We will load this onto our radiology system. Please send us copies if you can - -We do not routinely mail these back, but can do so if you request.
  • Appointment is scheduled (reception@dizziness-and-balance.com)
  • Prior to the consult, our office will call and obtain payment (via credit card). The amount is our usual bill to insurance for a "moderate complex" consultation. We do not bill insurance for telephone consults. You, however, may submit our invoice to your insurance if you feel that this is appropriate.
  • Appointment is carried out including, 10-20 minutes on review of records and questionnaire (off telephone) and 30 minutes on discussing situation with the patient. Please make sure we have your telephone #. We will call you. Depending on workload, sometimes we call a little later than scheduled.
  • Written report sent to patient within 2 weeks (contact transcription if you need it faster). It includes a summary of clinical situation and differential diagnosis, diagnostic recommendations and treatment recommendations.
  • Follow-up (reception@dizziness-and-balance.com): For "quick" questions, we will use email (no charge); For "long" discussions, we will ask you to schedule a followup telephone consult using a similar structure, You will be billed at the "followup visit" rate rather than "new patient"; Sometimes patients in whom a physical examination is essential to clarify the diagnosis, will come to Chicago after the initial telephone consult. This is also a "followup" visit.
